Referrals are required for Blue Care Network and some
Blue Choice Plans. We ask all our patients to contact their Insurance
Company for the following reasons:
- Duration of care (examples 60 calendar or 60 consecutive
visits per episode)
- Co-pay/deductible per visit (example: PHP requires a co-pay
per visit)
If you do not see your Insurance Company listed, we would be
considered out-of-network. In that case we will still bill your
plan, though there may be an out-of-network co-pay or deductible.
We ask that all of this be done prior to your first visit with
us at ARM, along with a prescription from the referring physician.
