Welcome! The first visit to our
office will be a comprehensive evaluation of the patient's condition
and will last approximately 1 hour. At that time, one of our experienced
therapists will determine the extent of the injury and the best
path to recovery.

Below are the required items to bring to the first appointment.
- Insurance Card(s)
- Drivers License
- List of Medications
- Health History/Allergies
- Insurance authorization if your provider requires authorization
before treatment.
- Prescription for therapy (or have it sent to us prior to
your appointment)
- Referring Physicians name, address, phone and fax numbers
Before therapy begins there are several
forms that must be completed. These forms provide us with the necessary
information to provide the most comprehensive care possible. Please
follow the links below to print out the forms needed prior to the
first visit or come 10-15 minutes before the appointment time and
complete them at our office.
